Gift Aid Form

For those members who have not already done so, please complete a gift aid form if you are UK taxpayer, paying an amount at least equal to the amount you are giving in income tax or capital gains tax.  This will allow us to reclaim tax at the rate of 25p in the pound, which makes a significant difference to any amount you give to us.

Use the button below to download the form to your computer, then print it off, complete and sign.  Please then email a scanned copy to the membership secretary foslmemsec@gmail.com or post it to:

Or you may give it to any of the FoSL Committee members who will pass it on to our Treasurer.
